How to Get Started
Please start by completing the Intake and Release Forms — then book your Initial Consultation to determine eligibility and treatment goals.
We require copies of supporting medical documentation including proof of diagnosis and pharmaceutical history (if applicable) before we can book your appointment with our physician.
Are you a Veteran? Get up to 3 grams / day covered by VAC.
